Hope's Insurance carrier for 2026 will be Blue Cross and Blue Shield of Illinois.

Open Enrollment is the only opportunity for employees to make changes to their optional healthcare plans without a qualifying life event.

​

Eligible employees may complete their individual enrollments for January 1, 2026, starting Monday November 17th in ADP. You can review summaries of the health plan options below.

Employees will see:

  • three health plan options

  • reductions in dental coverage costs

  • adjustments in disability costs

  • change in flexible spending providers. 

Health Option 1: Blueprint PPO

Individual deductible of $3000 with an individual out of pocket maximum of $6500

Family deductible of $9000 with a family out of pocket maximum of $18,400

Primary care visit: $35 copay

​

Specialist visit: $60 copay

​

Emergency Room visit: $150

Under this plan you do not need a referral to see a specialist.

​

Participant co-pays do not apply to the deductible.

BCBS Blueprint PPO

Summary of Benefits and Coverage

While this plan has the most expensive per pay premiums, participants do not need to navigate between two tiers and costs for in network providers. For employees local to Springfield, IL this plan includes Springfield Clinic providers without increases in co-pays and deductibles.

Health Option 2: Blue Choice PPO

This option provides employees with two tiers of in-network providers: Blue Choice Providers and Preferred Providers. Employees pay less when using providers who are in Tier 1 (Blue Choice). 

Tier 1: Blue Choice Providers

Individual deductible of $3000 with an individual out of pocket maximum of $5500

Family deductible of $9000 with a family out of pocket maximum of $9000

​

Primary care visit: $40 copay

​

Specialist visit: $65 copay​

Tier 2: Preferred Providers

Individual deductible of $4500 with an individual out of pocket maximum of $6500

​

Family deductible of $18,000 with a family out of pocket maximum of $18,000

​

​

Primary care visit: $65 copay

​

Specialist visit: $130 copay​

These tiers are specific to the state of Illinois. For employees local to Springfield, Illinois, Springfield Clinic Providers are currently Tier 2.

​

For employees outside of the state of Illinois, if your provider is in Blue Cross Blue Shields state network they will automatically fall into Tier 1.

BCBS Blue Choice PPO

Summary of Benefits and Coverage

Emergency Room: $500 copay plus 20% coinsurance after meeting the deductible.

​

Under this plan you do not need a referral to see a specialist.

​

Participant co-pays do not apply to the deductible.

While this plan allows for reduced costs in premiums, participants will need to navigate between two tiers and costs for in network providers. 

​

Example: If employees have already met their Tier 1 individual deductible of $3000 and then go to a provider who is in Tier 2, they will then have to spend an additional $1500 before meeting the Tier 2 individual deductible. 

Health Option 3: High Deductible Health Plan

Individual deductible of $5000 with an individual out of pocket maximum of $7000

Family deductible of $10,000 with a family out of pocket maximum of $14,000

Primary care visit: 20% coinsurance after deductible

Specialist visit: 20% coinsurance after deductible

​

Emergency Room visit: 20% coinsurance after deductible

BCBS High Deductible Health Plan

Summary of Benefits and Coverage

Under this plan you do not need a referral to see a specialist.

​

This plan offers employees the most affordable premiums for 2026. Employees in this plan do not need to navigate between two tiers or costs for in network providers. This plan does not allow for office visit copays as the employee pays the first $5000 in individual costs. After the deductible is met, the plan will pay coinsurance.

Employee Wellness 

Program Overview

The goal of the program is to reduce the potential for catastrophic health events through regular and routine health screenings. Employees who complete their annual physical (with labs) each year will be eligible for reduced healthcare premiums for enrollment the following year. In cases where employees cover their spouse on their insurance, both the employee and their spouse must complete the annual wellness form in the eligible timeframe.

The ONLY difference between a ‘wellness’ and ‘non-wellness’ plan is the per pay costs.

New or Newly Enrolled Employees

Employees who are new to Hope or newly enrolling in our healthcare plans will receive the discounted ‘wellness’ premiums. They will have a 60-day window from the effective date of their coverage to submit wellness documents to Human Resources to maintain those discounts.

​

To be eligible for discounts as a newly enrolled employee, the wellness and biometric screenings form must be dated within 12 months of the effective date of current healthcare coverage. Eligibility for future plan years will depend on the completion date of the wellness form.

Open Enrollment for 2026

To be eligible for enrollment in a 2026 Wellness plan, employees must have completed and submitted a wellness form to Human Resources between 11/1/2024 and 10/31/2025.​ Employees who enroll in a ‘wellness’ health plan, who have not satisfied the requirements for the 2026 plan year will have their premiums adjusted to the ‘non-wellness’ plan costs (a 10% increase).

​

Human Resources will audit employee selections and adjust plans in accordance with program participation.

Section 125 Cafeteria Plan

Employees enrolling in health, dental or vision coverage must choose between having their premiums deducted from their check on a pre-tax or after-tax basis. Employees who select the pre-tax option will pay less in taxes each paycheck, increasing their overall take home pay.

Please note, employees who select pre-tax options cannot cancel their healthcare coverage mid year without a qualifying life event.
